Diocese of Charleston COVID-19 Assumption of Risk, Waiver of Liability and Indemnity Agrement
If able, please print these and bring them to the church office along with a birth certificate and Baptism certificate for each child in a sacramental program.

Classes (K-6th grade) began on September 12 at 10:00 am. If your child is in a sacrament class (First Holy Communion or Confirmation), a long-form birth certificate and copy of their Baptism certificate (even if your child was baptized at Immaculate Conception) are required at the time of registration.
